• If you wish to cancel, please contact us by telephone during business hours the day before.
• Reservations are not required for “insurance treatment”. We will guide you in the order of your arrival.
• For first-time patients, please bring any of the following identification documents.
  (Insurance card, driving license, passport, individual number card, student ID card, etc.)
• Preferred dates and times are not confirmed for any counseling other than「Each counseling」. We will contact you after checking the availability of the slot.
• You can select your desired date from 「the next day」.
  Please note that requests sent outside of our clinic’s business hours will be confirmed and responded to after our clinic resumes business hours.
  It may also take a few days for us to reply. For emergency inquiries, please contact us by phone.
• If you are under 18 years old, you will need written consent from your guardian. Please print and fill out this form and bring it with you. (※Also available in our clinic)
